RENTAL SERVICE CONTRACT
Absolutely OXYGEN Telluride LLC.
Please read. This is your Contract. Your Supplemental Oxygen Service Supply Agreement, and Procedures.
Your booking/reservation/payment is considered by Absolutely Oxygen Telluride LLC
as your acceptance and agreement of the Supplemental Oxygen Supply Service Agreement Contract.
Supplemental Oxygen Concentrator UNITS are due for PICK UP from 10:00AM the day the Supplemental Oxygen Service Contract ends.
After delivery you are required to accept oxygen concentrator set up and safety instruction, and upon conclusion of this training/education you agree to immediately acknowledge by way of response text or email that you have received Oxygen Concentrator Safety and Usage Instruction. Your unwillingness to cooperate after AOTllc exhausting efforts to get this response will result in notice of intention to cancel your oxygen service without refund. The equipment will be picked up immediately from inside or outside of your property. AOTllc is a Phillips Respironics Approved Medical Oxygen Equipment Provider with Medical Equipment Industry Insurance and clear protocols to maintain client care and safety. Your understanding and cooperation is greatly appreciate.
PLEASE NOTE upon receipt of your oxygen equipment you will receive instructions and guidance. Rental terms require you to confirm in reply that you have received instruction and understand how to use the equipment . Failure to confirm your understanding will cancel your rental with full charge and the equipment will be collected immediately. Thank you for your support.

NEVER USE OXYGEN THERAPY NEAR FLAMES, IGNITING APPARATUS OR NEAR A SMOKER OR WHEN SMOKING.
OXYGEN THERAPY DESCRIPTION | Supplemental Oxygen Therapy is administered using an Oxygen Concentrator Unit and Breathing Cannula. Supplemental Oxygen Therapy is intended only for the prevention and/or relief of Altitude Sickness (medically diagnosed as Acute Mountain Sickness - AMS). Absolutely Oxygen Telluride LLC makes no medical claims and does not guarantee your cure of symptoms. Supplemental Oxygen Therapy administered using a nasal cannula has proven to be a successful method of treating the symptoms of AMS.
Supplemental OXYGEN THERAPY DAY AND NIGHT | To receive supplemental oxygen therapy, breath through the nasal cannula. You can do so as and when your symptoms dictate. During sleep, symptoms of AMS can increase and effect you more. While sleeping is a good time to use supplemental oxygen therapy. Ensure the cannula is fitted properly DO NOT place the hose around your neck. The Cannula hose rests on the ears and on the face. NEVER place the hose around your neck/head. Always supervise Children day or night using oxygen therapy.
BOOKING | Credit Card payments are processed in full at the time of reserving/booking your Supplemental Oxygen Service.
RESERVATIONS Made 24 Hours in advance are charged at standard published rental rate and will be delivered on scheduled day during published business hours. $40 Same Day Booking/Delivery incurs $40 Convenience Fee added to standard published rental rate.
AFTER HOURS BOOKING/DELIVERY Delivery AFTER HOURS between 9pm - 6am incurs $150 fee in addition to published rental rate.
CANCELLATION AFTER RESERVATION | Scheduled Oxygen Concentrator Rental Services with Delivery locks Absolutely Oxygen Telluride llc equipment into honoring your confirmed with payment booking. Equipment is set aside for your order and will not be offered to other Clients. Telluride is subject to dramatic and unpredictable weather patterns which can and will impact travel. Throughout the year flights may be cancelled and roads impassible. It is recommended that you add the cost of your Oxygen Service to your Travel/Trip Insurance.
Respectfully Cancellation Terms apply as follow without exception for whatever reason.
Bookings made of 10 days and over require 7 days notice to cancel, with a $200 administration charge deducted from refund.
More than 48 Hours notice - $120 deducted from original reservation payment. This takes care of time spent in consultation, equipment reserved and prepared for you.
Within 48 Hours - Cancellation within 48 hours of scheduled delivery. 60% of cost is charged and deducted from booking/reservation payment.
Within 24 Hours - Cancellation within 24 hours of scheduled delivery. 100% of the total cost is charged. There is no refund or credit offered.
SAME DAY - where Cancellation occurs day of scheduled delivery. 100% of the total cost is charged. No refund or credit offered. Even if just booked.
ALL Cancellations are subject to Credit Card Processing Fees deducted by Processing Company for amount of original payment and refund payment.

OXYGEN CONCENTRATOR UNIT DELIVERY | The Oxygen Concentrator Unit will be delivered between 12pm - 6pm the first day of the Supplemental Oxygen Service. While every attempt is made to be reliable, there may unforeseen mountain circumstances affecting efficiency. Please know that all seasons Telluride is subject to dramatic weather conditions, and with one road in and out of Telluride and Mountain Village, traffic can (rarely) backs up. Absolutely Oxygen Telluride LLC accepts no blame or liability for failure to deliver/supply oxygen concentration units, even when a fully paid booking/reservation exists. No compensation is offered.
RETURN OF OXYGEN CONCENTRATOR UNIT | Your Supplemental Oxygen Concentrator Service includes delivery and collection of the Oxygen Concentrator Unit to and from the address provided at time of delivery, and provided at the time of the booking/reservation. Please make the Oxygen Concentrator Unit available by 10am on the final day of rental - noted on your booking confirmation, and the reminder of the last day. Place accessories / instructions provided at delivery, under cover with the unit, cover provided at time of delivery, Return of Accessories and Undamaged Equipment is NOT the responsibility of Housekeeping, Hotel or Property Management. DAILY RATE CHARGE & RETURN PICK-UP CHARGE of $100 is applied.
BREATHING LINES - CANNULAS. USED & UNUSED CONSUMABLES, are disposed of. Cannulas purchased and unused are NOT refunded. 3/15/2020 PLEASE DISPOSE OF USED INDIVIDUAL CANNULAS / Breathing lines. RETURN Two Person airline connector (4 components) to zip bag.
PICK UP OF OXYGEN CONCENTRATOR UNIT | Please make the Oxygen Concentrator Unit available by 10am on the morning after the Supplemental Oxygen Service ends - detailed at the time of booking. Place accessories / instructions provided at delivery, place accessories under cover with the unit, cover provided at time of delivery,
RETURN / PICK UP - CLIENT RESPONSIBILITY | As a Client, you are responsible for making the Oxygen Concentrator Unit and Accessories available for full and prompt return on the due back date/time, detailed at time of booking or delivered with the unit. When the oxygen concentrator unit is not returned your Credit Card will be charged $75 fee for the Return To Pick Up service. Where the oxygen concentrator unit is not returned within 3 hours of the original Service completion and the Client has failed to communicate or respond to Absolutely Oxygen Telluride LLC's communication, a $145 Service Extension fee will be applied to the credit card used at the original Service booking/reservation. Where the oxygen concentrator unit is not returned within 24 hours of the Service completing, a $1250.00 fee to replace the Oxygen Concentrator Unit, accessories and cover the loss of Absolutely Oxygen Telluride LLC's Oxygen Service Business will be made to the credit card used for the original Service booking/reservation.
Return of Accessories and Undamaged Equipment is NOT the responsibility of Housekeeping, Hotel or Property Management. Pack Oxygen Unit and Accessories to avoid charges for missing items. Damage to Oxygen Concentrator Unit and loss of Unit Accessories - filter/hose door, dial, flow meter, wheels, unit casing, cable, tarp bag, 2 person connector etc, will incur charges to replace or repair damage, plus a $50 fee, to the credit card provided at time of booking/reservation.

Absolutely Oxygen Equipment and Accessories arrived with you in complete and pristine condition. Where equipment, and or accessories are damaged or missing. The following charges will be made to the credit card used at booking/reservation.
PHILIPS RESPIRONICS EVERFLOW and Q Version $2000 Fee.
Oxygen Concentrator Manual provided - damaged or not returned incurs a $30 Fee.
Canvass Zip Wallet, attached to unit, housing Manual and 2 Person Y Connector - not returned incurs $15 Fee.
Two Person Y Splitter and hose provided - damaged or not returned incurs a $65 Fee.
Oximeter provided at delivery - damaged or not returned $60 Replacement Fee.
Oximeter clip/lanyard - damaged or not returned $15 Replacement Fee.
Water Humidifier Bottle - damaged or not returned incurs a $30 fee.
Neoprene Cover for Water Humidifier Bottle - damaged or not returned incurs a $15 fee.
Humidifier Hose or Bottle Screw, attaches Humidifier Bottle to Concentrator Unit - any part of the 2 Part Hose damaged or not returned incurs a $60 fee.
Air Intake Vent Cover, attached to intake grill - damaged or not returned incurs a $75 fee.
Concentrator Filter - missing incurs $60 fee.
Blue Tarp Oxygen Concentrator Unit Cover provided - not returned incurs a $20 fee.

Damage and/or loss to Supplemental Oxygen Concentrator Unit and Unit Accessories - hose door, dial, gage, wheels, unit casing, cable etc, will incur charges of cost to replace or repair plus $50 fee, to the credit card provided at time of booking/reservation. Notice will be provided to contact provided at time of delivery.
Credit Card Processing Fees are deducted from refunds @ 3.75% + 15 cents per transaction.
ACCEPTED PROCESS OF RETURN |
If you plan to leave ahead of your scheduled end of Oxygen Service date, please advise so equipment is not left outside overnight, or becomes locked in residence. This could incur charges as below.
The Oxygen Concentrator and Accessories have to be available for collection by 10am on the final day of service. We DO NOT ENTER/TRESPASS roam a property looking for missing items. Place the Absolutely Oxygen Telluride LLC Oxygen Concentrator Unit with Accessories back under the cover provided at time of delivery. Place the covered Oxygen Concentrator Unit with the accessories and instructions, just inside the unlocked front door, or set back (out of weather) of the residence porch, identified as the delivery/pick up address, at booking. If a Hotel Guest, return the Oxygen Concentrator Unit with Accessories to the Hotel's Front Desk, Bell or Concierge Stand and make it known via text, email or telephone call to Absolutely Oxygen Telluride LLC.
When an Absolutely Oxygen Telluride LLC Concentrator Unit is not available for the agreed pick up, issued at the time of Service reservation, this will incur a $75 Return to Pick Up Fee, and/or after 3 hours of failing to return or communicate with Absolutely Oxygen Telluride LLC, a 'With No Prior Agreement Rental Extension fee of $145 Per Night will apply. These terms and fees are non negotiable. Fees will be applied to the Credit Card provided at time of booking/reservation. Notice of these charges will be sent to the number used at the time of booking/reservation.
RESERVATIONS MADE BY AN AGENT - Third Party |
Where a reservation is made by an Agent (Third Party), such as an Event Planner, Property Manager, Travel Agent, Personal Assistant, Business Manager, Concierge, Hotel Front Desk, Relative or similar. As the Third Party Agent you are responsible for the reservation, and any additional charges and/or accessory replacement and/or repair fees due to Absolutely Oxygen Telluride LLC. This includes, and is not limited to credit card chargebacks and all associate fees.
Please confirm with your client, and gain full and final approval from your client, that you are endorsed as their Agent, with their permission to represent them in transactions and contracts with Absolutely Oxygen Telluride LLC. When making a reservation as a Third Party Agent you are confirming that you unreservedly have your client's full consent and accept all Absolutely Oxygen Telluride LLC's Terms and Conditions of Contract, on their (your clients/end users's) behalf, and that you have made your client/the end user aware of all associated costs. as well as this Absolutely Oxygen Telluride LLC Terms and Conditions of Contract document.
MEDICAL OXYGEN EQUIPMENT SALES - PRESCRIPTION ONLY
Federal Law requires that a Physician written Prescription is required to purchase medical oxygen equipment. Sales Tax is Zero. The prescription must be issued in the Purchaser/Client's name. If it is discovered at a later date that the Purchaser/Client's name is different to the name noted on the Prescription provided (the Purchaser/Client has just changed their name etc), the Purchaser/Client must provide formal documented proof that the name provided is a name the Purchaser/Client has legally used. In instances where this discrepancy lingers, when the Purchaser/Client cannot or is not willing to resolve the discrepancy, the Purchaser/Client will be required to make the same equipment and all items/accessories available for collection and return to Absolutely Oxygen Telluride llc. These are the Purchaser/Client's accepted terms of sale at point of sale/payment. When a reasonable and logical resolution has not been accepted by the Purchaser/Client and return is deemed necessary by the Seller Absolutely Oxygen Telluride llc. The Purchaser/Client's original payment amount will be deducted of fees for the original delivery, service to install/train at Purchaser/Client's residence, check equipment before loading for return, pick up, store restocking, and *rental service* fees for the period of time the Purchaser/Client has had the equipment in their (Purchaser/Client's) possession. *Two days rental fee will added/charged to rental fee for day of delivery and pick-up*. NO REFUNDS PAID. Absolutely Oxygen Telluride llc will issue Credit for the balance in Purchaser/Client's name, to be used by Purchaser/Client within 18 months of the return/pick up date. Thank you.
Absolutely Oxygen Telluride LLC does not subcontract to any other Supplemental Oxygen Service Company.
Absolutely Oxygen Telluride LLC is not a Medical Provider.
Absolutely Oxygen Telluride LLC does not accept Medical Insurance as payment for Services.
Absolutely Oxygen Telluride LLC does not accept Medicare or Medicaid.
Absolutely Oxygen Telluride LLC a non-participating entity and does not accept assignment.
Absolutely Oxygen Telluride LLC does not offer delayed or credit term payments.
Please note you are the Client and Absolutely Oxygen Telluride LLC is the Oxygen Service Provider, and upon making your Oxygen Concentrator Service booking and payment, you do so having fully read, understood and accepted the Terms and Conditions of Oxygen Service Contract, provided here on this Absolutely Oxygen Telluride LLC website.
